Request for portabilty

Data portability is your right to request that computerized personal information you have entrusted to us and that we have stored be communicated to you or to an authorized organization. This right comes into effect on 22 September, 2024. Centraide of Greater Montreal reserves the right to reject a request if it raises serious technical difficulties, particularly if it is too complex, involves significant costs or if we are unable to identify you.

PORTABILITY REQUEST PROCEDURE

  • Fill in and send the form.
  • We will contact you to verify your identity, and if required, to clarify your needs. With some exceptions, your identity will be verified by videoconference.
  • Additional questions could be asked in order for us to find the required information in our systems.
  • Your request will be processed within 30 days.

The information collected through this form is required by Centraide to process your request (information or contact request) to follow up on it and communicate with you. Your information will be communicated to our subcontractors and partners to whom it is necessary to communicate it for these purposes, such as our Internet providers and related applications, which may be located outside Quebec. You have the right to access and rectify your personal information, and you may withdraw your consent at any time. By providing your information on this form, you acknowledge that you have consulted Centraide of Greater Montreal’s privacy policy.
